Destination Florida - The North of England Children's Charity

DO YOU KNOW A POORLY CHILD WHO WOULD LOVE TO GO DISNEYLAND?
If you wish to nominate a child for this year’s Holiday of a Lifetime to Disneyland in Florida from 28th October to 6th November, please complete this form and send it to the Destination Florida office.
The criteria for a place on the trip are as follows and each nomination is carefully considered on an individual basis by the Medical Committee and Trustees of Destination Florida. The completion of the form does not guarantee a place on the trip. We kindly point out we receive many applications for places on the trips to Florida.
The criteria are:

The children should be aged 7 to 15years and living in the North West Region of England.
Not to have visited Disneyland in Florida previously either with family or another charity.
Be aware of surroundings and have a mode of communication.
Diagnosis of a life threatening and/or life limiting condition, terminal illness or other challenging health condition.
Children are not accompanied by their families, guardians or carers.

Anyone can complete a Child Nomination Form but it must be signed and verified by a Doctor before sending to the Destination Florida office for consideration of a place on the trip.
